Pre-Qualification

If you are interested in being added to the waiting list, you must first complete a pre-qualification form. To download the pre-qualification form right-click here and choose 'save target as' on the pop-up menu. After completing the form please send it as an attachment to form@dogtownstation.com. Upon verification, you will be added to the waiting list.
